Responsibilities

Must be fully bilingual in English and Spanish

This position requires travel between North Miami, Little Havana and Hialeah to cover the center needs

Conviva is seeking a candidate to join our Team as an ACCESS Representative , who is interested in being part of a team that focuses on excellence and service to others.

Location Schedule - Required to work out of the following centers throughout the week:

Conviva Care Center North Miami Beach: 2 days out of the week

1648 NE 163rd Street

North Miami Beach, Florida 33162

Conviva Care Center Little Havana 3 days out of the week 1200 SW 1st St, Miami, FL 33135

Conviva Care Hialeah Centers as needed

Essential Functions:


Addresses and document inquiries by following department process and Department of Children and Families (DCF) guidelines.
Serves as advocate and educator to applicants in need of assistance of services provided by DCF.
Travels/works out of multiple Conviva centers.
Comply with all HIPPA and rules and regulations.
Maintain applicant files and follow up on case status.
Provide weekly and monthly report to direct supervisor.
Contribute to team effort by accomplishing related results as needed


Required Qualifications


Bilingual English and Spanish
Knowledge of Microsoft Word, Excel and Outlook
Administration support experience in a healthcare industry
Strong customer service skills
Attention to detail
Must be willing to travel locally to work out of multiple Conviva centers.
Effective verbal and listening skills
Ability to multi-task, prioritize and manage time effectively.


Preferred Qualifications:


High School Diploma
Experience working with DCF and/or the DCF ACCESS program
Experience resourcing assistance programs to help low-income households and/or better the lives of others.
Social worker or case management experience
Microsoft Access proficiency


Monday through Friday: 8am to 5pm

Additional Information :

Associates are required to be fully COVID vaccinated, including booster, or undergo weekly COVID testing and wear a face covering while at work. The weekly testing will need to be done through an approved Humana vendor, and unvaccinated associates should follow all social distancing and masking protocols if they are required to come into a Humana facility or work outside of their home.
