ABOUT THIS TEMP POSITION
This is a temporary position and the length of assignment is estimated to go from 7/2022 to 10/2022. The length of the assignment is always dependent on business need and dates may change. While the assignment would be at the Alliance, if selected, you would be an employee of a temporary employment agency that we would connect you with.
WHAT YOU'LL BE RESPONSIBLE FOR
We are looking for both Pediatric and Enhanced Care Management (Adult) Care Coordinators.
The Pediatric Care Coordinators would be responsible for:
Conducting telephone interviews with members, significant others and family members to determine if care needs are being met or if additional services are needed
Ensuring that members make and keep scheduled medical appointments
Facilitating prior authorization of services with provider offices and community agencies
Identifying gaps in care for members and arranging appropriate and timely resolution with internal and external agencies
Scheduling assessments, coordinating and obtaining medical records and authorizations and ensuring processes are completed within state required timelines
Utilizing the Alliance computer system to complete tasks, update care plans, assessments and interventions
Attending Care Rounds as assigned and contributing updates to member’s care plans, making recommendations for improvement in outcomes and following up with interventions and tasks as assigned
May be assigned to back up Care Coordination - Intake Coordinator in reviewing and assigning internal and external referrals to appropriate staff
May assist staff with transcribing clinical documentation
The Enhanced Care Management (Adult) Care Coordinators would be responsible for:
Conducting telephone interviews with providers, members, and family members to determine if criteria are met for member to receive services
Obtaining and/or entering authorization requests for services with follow up to ensure and coordinate delivery, such as ensuring appropriate contact information is available for providers to conduct outreach to the member
Facilitating prior authorization of services with provider offices and community agencies
Coordinating and obtaining medical records, as needed, and authorizations and ensuring processes are completed within state required timelines
Utilizing the Alliance computer system to complete tasks and review appropriate information
Attending meetings as assigned and contributing updates to member’s care plans, and following up with pertinent information and tasks as assigned
Creating new authorizations, thoroughly documenting and monitoring notes in the Alliance computer system
May assist staff with transcribing clinical documentation
Communicating, both orally and in writing, with members, providers, and internal stakeholders
Performing administrative duties to track, organize, monitor and follow-up on case work
Tracking receipt, assignment, enrollment and disenrollment of cases/authorizations
Monitoring electronic queues to ensure requests are entered and assigned appropriately
WHAT YOU'LL NEED TO BE SUCCESSFUL

Knowledge of:

Diverse needs of the Medi-Cal population
Bilingual (English/Spanish) is preferred but not required
Ability to:
Understand and communicate the workings of the Alliance, particularly the Utilization Management and Complex Case Management, Community Care Coordinator, and Member Services departments
Competently navigate computer systems
Use Microsoft Word and Excel, and Alliance computer systems
Work independently with minimal supervision
Verbally communicate the program mission, vision, and roles
Education and Experience:
High school diploma or equivalent and three years (or an Associate's degree in health, social services or a related field or Medical Assistant certification and a minimum of two years) of relevant experience in a health care setting interacting with members, patients and/or providers in meeting service needs; or a combination of education and relevant work experience may be qualifying
OTHER DETAILS
While this position is connected to one of our Alliance offices, we are in hybrid telecommute work environment right now and we anticipate that the interview process will take place remotely.
Our Alliance office locations have officially re-opened as of May 2, 2022 and while some employees may work in full-time telecommute schedules, attendance at quarterly company-wide events or department meetings will be expected.
Based on the nature of work, this position may require onsite presence, which is dependent on business need. Details about this can be reviewed during the interview process

COVID-19 Vaccine Requirement:
This position requires staff to be fully vaccinated and boosted upon their start date as a condition of employment. Proof of vaccination must be submitted at time of hire. Fully vaccinated and boosted is defined as receipt of the second dose in a 2-dose series (Pfizer & Moderna vaccines) OR receipt of a single-dose vaccine (Johnson & Johnson); AND receipt of the booster (if eligible); AND a 2-week period having passed since the administration of the last vaccine dose.
