The Care Coach 1 assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Coach 1 work assignments are often straightforward and of moderate complexity.
Responsibilities
This is an office based position in Orlando, FL. Initial training will be 6-8 weeks virtually and temporary WAH and maybe eventually in the Orlando office.
This position also requires home visits to members in the area and surrounding counties as permitted.
The Care Coach 1 employs a variety of strategies, approaches and techniques to manage a member's health issues. Identifies and resolves barriers that hinder effective care.
Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through use of assessment, data, conversations with member, and active care planning.
Understands own work area professional concepts/standards, regulations, strategies and operating standards.
Makes decisions regarding own work approach/priorities, and follows direction. Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation.
Required Qualifications
Bilingual English/Spanish - must be able to speak, read and write in both languages without limitations or assistance
Must be able to work Monday through Friday between the hours of 8am-7pm but be flexible with your hours based on business needs and hours could include weekends/holidays during the Medicare Annual Election Period (Sept 16-Dec 8)
Previous experience in phone intensive environment with strong customer service skills both inbound & outbound (70-80% at times)
Previous administrative or related experience
Current valid Driver's license required & ability to travel up to 75% at times in surrounding counties
Team player, ability to work with tight schedules, multiple priorities as well as work independently
Strong proficiency in Microsoft Office Programs Word, Excel
Must be passionate about contributing to an organization focused on continuously improving consumer experiences

This job must be in the office and requires close personal interaction with patients. You will not be able to social distance.

Preferred Qualifications
Associates /Bachelors
Prior experience with Medicare & Medicaid recipients
Previous experience with electronic case note documentation and experienced with documenting in multiple computer applications/systems
Experience with health promotion, coaching and wellness
Knowledge of community health and social service agencies and additional community resources
