Description

The Certified Diabetic Care and Education Specialist (CDCES) delivers comprehensive and seamless nursing services that bridge the gap and integrate clinical and self-management aspects of diabetes care. The CDCES is an integral part of the Multi-Disciplinary Care Team and provides collaborative, comprehensive and member-centered care, and education conducive to behavior change and improved quality of life across the lifespan. The CDCES supports and advocates for members to optimize quality care. The CDCES promotes self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes.

Responsibilities

Humana is seeking a Registered Nurse who is a Certified Diabetes Care and Education Specialist for our Care Management Team to provide guidance to members managing Diabetes. The RN CDCES will provide information, resources and tools to increase the member's ability to manage their own condition and enable them to better understand their health care options and become active decision makers with their clinical treatment team. In this role, the RN CDCES:

Advocates for and communicates about improved quality of care and outcomes for those living with diabetes and other comorbidities.

Actively participates in the quality improvement process and adapts practice/process based on findings.

Understands clinical program design, implementation, and management, monitoring and reporting.

Integrates skills and knowledge of pathophysiology, epidemiology, clinical management, cardio metabolic conditions, and self-management of diabetes into clinical practice.

Provides quality diabetes self-management education in virtual and/or telephonic individual and group settings based on member needs and preferences. Utilizes appropriate teaching techniques that are sensitive to the learning preferences of the person with diabetes.

Follows the ADCES Diabetes Education Accreditation Program requirements which are based on the National Standard of Diabetes Self-Management and Support Guidelines.

Completes comprehensive assessments for each member including emotional and behavioral health, interprets personal health data, develops an individualized care plan based on the member's assessed needs and goals and promotes successful self-management.

Collaborates, advocates, and confers with other members of the Multi-Disciplinary Care Team in developing member-centered diabetes plans.

Advocates for and supports technology-enabled diabetes education and care, in individual and population health diabetes services.

Performs other duties as assigned.

Travel may be up to 5%

Required Qualifications

Certified Diabetes Care and Education Specialist.

Active RN (Registered Nurse) license in eNLC state with no disciplinary action and without restrictions.

Resides in an eNLC state with active multistate licensure.

Experience with rollout of Diabetes program

3 years or more in an acute care setting of clinical nursing experience.

Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook

Exceptional communication and interpersonal skills.

Ability to work independently under general instructions and with a team.

Strong computer, web navigation and research skills.

Ability to effectively navigate virtual information systems.

Must have a separate room with a locked door that can be used as a home office to ensure you and your patient has absolute and continuous privacy while you work.

Must have accessibility to high speed DSL or cable modem for a home office. Please note Satellite internet cannot be used for this role. Internet speeds must be at a minimum of 25Mbps Download by 10Mbps Upload for Cable Broadband

Ability to work a full-time (40 hrs minimum) work schedule with availability to work between the hours of 8:30 a.m. and 7 p.m. to meet business needs.

Preferred Qualifications

Bilingual in Spanish is highly desired

Call center or telephonic triage experience

Has working knowledge of diabetes technology.

Experience with case management, discharge planning and patient education for adult acute care.

Additional Information

For this job, associates are required to be fully COVID vaccinated or undergo weekly COVID testing and wear a face covering while at work. The weekly testing will need to be done through an approved Humana vendor, and unvaccinated associates should follow all social distancing and masking protocols if they are required to come into a Humana facility or work outside of their home. Interview Process

If participating in a SMS Text interview, you will be asked a series of questions to which you will be using your cell phone or computer to answer the questions provided. Expect this type of interview to last anywhere from 5-10 minutes. Your recorded interview(s) via text and/or pre-recorded voice will be reviewed and you will subsequently be informed if you will be moving forward to next round of interviews.
