Description

The Care Coach 1 assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Coach 1 work assignments are often straightforward and of moderate complexity.

Responsibilities

The Care Coach 1

Employs a variety of strategies, approaches and techniques to manage a member's health issues.

Identifies and resolves barriers that hinder effective care

Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through use of assessment, data, conversations with member, and active care planning

Understands own work area professional concepts/standards, regulations, strategies and operating standards

Makes decisions regarding own work approach/priorities, and follows direction

Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation.

This is a temporary Work-At-Home position that will eventually be based back in our Daytona Beach office.

Initial training will be 4-6 weeks, mostly virtually and attending some trainings in person, as allowed.

Once covid-19 related restrictions are lifted, this position will also require home visits to members and to events in the Daytona Beach area and surrounding counties.

Required Qualifications

Bilingual English/Spanish with ability to pass a language assessment

Ability to work Monday - Friday between the hours of 8am-7pm (with flexibility based on business needs, including weekend/holidays, specifically during Medicare Annual Election Period (Sept 15-Dec 8)

Previous experience in phone intensive environment, inbound and outbound (90%, but flexible)

Strong customer service skills

1 year of administrative or related experience

Current valid Driver's license required & ability to travel up to 75% at times in surrounding counties

Team player, ability to work with tight schedules, multiple priorities as well as work independently

Strong proficiency in Microsoft Office Programs, i.e. Word, Excel

Must be passionate about contributing to an organization focused on continuously improving consumer experiences

Must be reliable and adhere to attendance policies.

Preferred Qualifications

Associates degree

Prior experience with Medicare & Medicaid recipients

Previous experience with electronic case notes and documenting in multiple computer applications/systems

Experience with health promotion, coaching and wellness

Knowledge of community health and social service agencies and additional community resources
