Responsibilities
The Humana At Home Care Coach 1 will telephonically survey Medicare, Commercial or Group Account members in 7 domains of functioning. Evaluate member needs and requirements to achieve and/or maintain optimal wellness state. Guide members and their families towards and facilitate interaction with resources appropriate for the care and wellbeing of members. Work in collaboration with a multi-disciplinary team, employing a variety of strategies, approaches and techniques to manage member physical, environmental and psycho-social health issues.
Assess and follow through on basic member needs
Navigate community resources and follow-up on resource availability
Provide telephonic support to Humana staff or members as they navigate community resources
Skill to leverage appropriate resources based on the intervention(s) and members needs
Work collaboratively with other members of the Humana Interdisciplinary team
Understand clinical program design, implementation, and management, monitoring and reporting
Comply with performance and reporting standards as defined by Humana Corporation
Additional responsibilities as deemed appropriate by Humana leadership
Provide guidance regarding applications and/or processes related to gaining access to resources
Identify available social and community resources
Additional responsibilities as deemed appropriate by Humana At Home
Role Essentials
Associates degree in Behavioral Health or Health related field AND
Minimum three years' experience in social services or a health related field.
Knowledge of community resources and any health or social service agencies
Experience using talking points to explain eligibility and use of services
Exceptional communication and interpersonal skills
Ability to interact effectively with multi-disciplinary team members
Excellent keyboard and web navigation skills
Intermediate to advanced computer skills and experience with Microsoft Word and Excel
Ability to use a variety of electronic information processing tools
Self-starter who is able to multi-task and prioritize
Must have a separate room with a locked door that can be used as a home office to ensure you and our members have absolute and continuous privacy while you work
Must have accessibility to high speed DSL or Cable modem internet for your home office (Satellite internet service is NOT allowed for this role); and recommended speed for optimal performance form Humana systems is 10M X 1M
Ability to work a full-time (40 hours minimum) Monday - Friday
Role Desirables
Bachelor's degree in social, behavioral, or health related field
Experience with behavior change, health promotion, coaching and wellness
Previous managed care experience
Previous work with vulnerable adults or the geriatric population
Knowledge of scope and activities of public and private health agencies
Previous telephonic case management experience working in the medical/insurance field
Basic knowledge of complex care management and care management principles
Experience with motivational interviewing
Bilingual (English/Spanish); speaking, reading, writing, interpreting and explaining documents in Spanish
Central and mountain time zones would be a plus

Our business calls members and/or caregivers at the best times possible to support their health and wellbeing. You must be able to work a flexible full time schedule, which includes Mon-Fri and sometimes Saturdays for 40 hours a week minimum.
During the first 90 training days the schedule is typically between 8:00am - 5:30pm M-F, depending on member and current business needs
After completion of training, you and your leader will create a 5-day work schedule that typically falls between the hours of 8:00am - 8:00pm M-F and sometimes Saturdays, depending on member and current business needs
